Lawmaker Cesia Rivas of Vamos called on Wednesday for the initiation of an interpellation process against the director of the Salvadoran Social Security Institute (ISSS), Mónica Ayala Guerrero, so she can account for the institution’s financial situation and the causes of delays in medical appointments and sick leave payments.

The request was not added to the plenary session agenda, as it received only three votes — one from lawmaker Rivas and two from the Nationalist Republican Alliance (Arena).

Rivas questioned the fees the ISSS charges workers every month and the deficient service provided by ISSS health centers.

“That mandatory fee deducted month after month from every employee, every worker in El Salvador — it’s there, month after month. But despite that punctuality, they sure make Salvadorans who use Social Security wait,” she said.

The lawmaker added that appointments for the ISSS Orthopedics Department are scheduled for February 2027. “Anyone going to Orthopedics is there because they’ve suffered a fracture that should be treated urgently — appointments months away, surgeries on hold. There are no medications, no specialists. And on top of that, sick leave isn’t being paid,” she noted.

She cited the case of a worker from Cojutepeque, in the department of Cuscatlán, who after two months still has not been paid her sick leave. “I heard from a worker in the financial sector who had already gone four months without being paid for his sick leave,” she indicated.

The lawmaker cited a journalistic investigation to point out that the ISSS spent $152 million on outsourced services and $24 million on infrastructure. “Spending on third-party contracts is six times greater than what the institution invests in its own facilities,” she criticized.

According to the lawmaker, the official should answer the following questions:

• How many sick leave payments are currently pending, and what is the total amount owed?
• What is the average wait time for sick leave payments, and how many cases exceed 30, 60, and 90 days of delay?
• What are the causes of the delays in sick leave payments, and what measures are being implemented to resolve them?
• What is the average wait time to obtain a general medicine appointment and a specialist appointment?
• How many patients are currently on waiting lists for specialized care?
• Which medical specialties have the longest delays in appointment scheduling?
• How many surgeries are currently pending across the ISSS hospital network?
• How many patients have been waiting more than six months for a surgical procedure?
• What institutional plan exists to reduce the surgical backlog and wait times?
• How many specialists have left the ISSS from 2019 to date?
• How many specialist positions are currently vacant, and which areas are most affected?
• What strategies are being implemented to hire and retain medical specialists?
• What is the current financial situation of the ISSS, and what is the amount of its institutional reserves?
• How much money has the ISSS received in contributions over the past five years?
• What has been the annual spending on outsourced services over the past five years, and what is the justification for that spending?
• What is the current amount of employer arrears, and what actions are being taken to recover those funds?
• What has been the investment in hospital infrastructure, medical equipment, and service expansion over the past five years?
• What is the current state of medical equipment in ISSS hospitals and clinics?
• What financial and administrative audits have been conducted over the past five years, and what were the main findings?
• Do you believe that Salvadoran workers are receiving services commensurate with the resources they contribute to the system each month?

The most recent financial statement published by the ISSS showed a financial deficit of -$64 million (negative) at the close of 2024, which shrank to -$6 million (negative) in 2025 after ISSS revenues rose from $827 million to $866 million. Most of the increase came from “Social Security revenues,” which went from $799 million to $841 million.

The deficit reduction also followed a cut in expenditures from $891 million to $872 million, mostly in personnel costs, which dropped from $436 million to $417 million.

Diputada de Vamos pide interpelar a directora del ISSS por atrasos en citas médicas y situación financiera

La diputada Cesia Rivas, de Vamos, pidió este miércoles que se inicie un proceso de interpelación a la directora del Instituto Salvadoreño del Seguro Social (ISSS), Mónica Ayala Guerrero, para que rinda cuentas sobre la situación financiera de la institución y las causas de los retrasos en las citas médicas y en los pagos de las incapacidades.

La solicitud no ingresó a la agenda de la sesión plenaria, ya que únicamente tuvo tres votos, uno de la diputada Rivas y dos votos de Alianza Republicana Nacionalista (Arena).

Rivas cuestionó el cobro que el ISSS hace a los trabajadores todos los meses y la deficiencia del servicio que prestan los centros de salud del ISSS.

“Esa cuota obligatoria que mes a mes se descuenta a todo empleado, a todo trabajador de El Salvador, ahí está, mes a mes. Pero, a pesar de esa puntualidad, a los salvadoreños que hacen uso del Seguro Social vaya que los hacen esperar”, dijo.

La diputada agregó que las citas para el Departamento de Ortopedia del ISSS están programadas para febrero de 2027. “El que va a Ortopedia es porque ha sufrido una fractura que debería ser urgente, citas a meses, cirugías en espera. No hay medicamentos, no hay especialistas. Y, además de eso, no se les está pagando las incapacidades”, señaló.

Citó el caso de una trabajadora de Cojutepeque, en el departamento de Cuscatlán, a quien luego de dos meses no le han pagado su incapacidad. “Escuchar a un trabajador en el sector financiero, llevaba ya cuatro meses donde no se le había pagado su incapacidad”, indicó.

La diputada citó una investigación periodística para indicar que el ISSS tuvo un gasto de $152 millones en servicios externos y $24 en infraestructura. “El gasto en contratación de terceros es seis veces mayor que lo que la institución invierte en sus propias instalaciones”, criticó.

Según la diputada, la funcionaria debería responder estas preguntas:

• ¿Cuántas incapacidades se encuentran actualmente pendientes de pago y cuál es el monto adeudado?
• ¿Cuál es el tiempo promedio de espera para el pago de incapacidades y cuántos casos superan los 30, 60 y 90 días de retraso?
• ¿Cuáles son las causas que ha provocado los retrasos en el pago de incapacidades y qué medidas se están implementando para solucionarlos?
• ¿Cuál es el tiempo promedio de espera para obtener una consulta de medicina general y una consulta especializada?
• ¿Cuántos pacientes se encuentran actualmente en listas de espera para atención especializada?
• ¿Cuáles son las especialidades médicas con mayor retraso en la asignación de citas?
• ¿Cuántas cirugías se encuentran actualmente pendientes de realización en la red hospitalaria del ISSS?
• ¿Cuántos pacientes tienen más de seis meses esperando una intervención quirúrgica?
• ¿Qué plan institucional existe para reducir la mora quirúrgica y los tiempos de espera?
• ¿Cuántos médicos especialistas han dejado de laborar en el ISSS desde 2019 a la fecha?
• ¿Cuántas plazas de especialistas se encuentran vacantes actualmente y cuáles son las áreas más afectadas?
• ¿Qué estrategias se están implementando para contratar y retener médicos especialistas?
• ¿Cuál es la situación financiera actual del ISSS y cuál es el monto de sus reservas institucionales?
• ¿Cuánto dinero ha recibido el ISSS en concepto de cotizaciones durante los últimos cinco años?
• ¿Cuál ha sido el gasto anual en contratación de servicios externos durante los últimos cinco años y cuál es la justificación de dicho gasto?
• ¿Cuál es el monto actual de la mora patronal y qué acciones se realizan para recuperar esos recursos?
• ¿Cuál ha sido la inversión en infraestructura hospitalaria, equipamiento médico y ampliación de servicios durante los últimos cinco años?
• ¿Cuál es el estado actual del equipamiento médico en hospitales y clínicas del ISSS?
• ¿Qué auditorías financieras y administrativas se han practicado en los últimos cinco años y cuáles han sido principales hallazgos?
• ¿Considera que los trabajadores salvadoreños están recibiendo servicios acordes con los recursos que aportan mensualmente al sistema?

El más reciente balance financiero publicado por el ISSS mostró un déficit financiero de -$64 millones (negativo) al cierre de 2024 que disminuyó a -$6 millones (negativo) en 2025, luego de que los ingresos del ISSS aumentaran de $827 a $866 millones, la mayor parte del aumento ocurrió en «ingresos de Seguridad Social», de $799 a $841 millones.

La reducción del déficit también ocurrió luego de un recorte en los gastos de $891 a $872 millones, en su mayoría en gastos de personal, de $436 a $417 millones.
