The Union of Medical Workers of the Salvadoran Social Security Institute (SIMETRISSS) said that the merger between the institution and the Ministry of Health (MINSAL) is already happening, asserting that at Amatepec Hospital in Soyapango, appointments are being scheduled and procedures processed for patients arriving from the public sector.
“It can be said that the merger is already underway. A year ago they said no, that there was no merger, but today we see the presence of the Ministry of Health in the emergency room of Amatepec Hospital, where Ministry of Health paperwork is already being handled for patients,” said Dr. Rafael Aguirre, secretary general of SIMETRISSS, in a June 17 interview on the Informa TV program.
The union leader explained that at Amatepec Hospital, staff are already using the Integrated Health System, a MINSAL IT program for patient care. He added that the merger is not 100% complete, as the “right” to be treated at the Social Security Institute is still being checked.
“Right now, the emergency room at Amatepec Hospital has implemented the Integrated Health System, which is a computer system that the Ministry (of Health) has for serving the population. Eligibility is still being checked—that is, whether the person is entitled to Social Security benefits or not—but once that is removed, we can say the merger is complete,” the doctor added.
The SIMETRISSS representative explained that eligibility refers to whether a person is a contributor or beneficiary of the Social Security Institute, as a payroll deduction is made (in the case of the contributor), and therefore they should be treated at any clinic or hospital of the institute. However, at Amatepec Hospital, patients who are not enrolled in the institute are being seen.
“People are bringing paperwork (appointments, lab orders, and other documents) to the hospital (Amatepec) and the first thing our colleagues say is ‘but that order is from MINSAL,’ so what they did was distribute a memorandum stating that if people arrive with that paperwork, not to deny them appointments, but to schedule them,” Dr. Aguirre stressed.
“We see that this (MINSAL) paperwork is already being used and it has been requested that when patients arrive to process ultrasounds, X-rays, or bring prescriptions with the Ministry of Health logo, they should be assisted and given appointments,” he stated.
For the SIMETRISSS leader, this is already a form of merger between the two institutions, although neither has officially confirmed it.
“That, in practice, is an already merged part of both institutions because if people with MINSAL-related matters can go get an appointment and the Social Security Institute is processing it, then that is already being merged. The only thing missing for them to say a merger has occurred is to remove the requirement of checking a patient’s social security eligibility,” the specialist added.
He stated that at the moment, only Amatepec Hospital is receiving patients who are not part of the Social Security Institute for appointments or other procedures, clarifying that medical care at the facility is, for now, provided only to beneficiaries, meaning contributors and their dependents.
He also explained that this type of merger will eventually be implemented at the Santa Tecla Medical Unit and the Zacamil Polyclinic Hospital.
On April 15, the Foro Nacional de Salud (FNS) (National Health Forum) commented on the merger of the ISSS and MINSAL, stating that its objective is to reduce costs and that it does not guarantee the population will receive good service.
“If they are going to merge the health system, MINSAL with the Social Security Institute, I see it more as a way to save on personnel and everything else, not to improve care. And if they are going to improve it, it should be with international quality standards, not just to merge and reduce costs, but to guarantee care for the population,” said Morena Murillo of the FNS.
At that time, Murillo explained that an ISSS-MINSAL merger would require defining which institution would handle health promotion, a responsibility that currently falls to the public sector.
“The Social Security Institute does not carry out health promotion or disease prevention work. Seventeen percent of the population is served by the Social Security Institute, so that would be burdened onto MINSAL, which would not have the resolutive capacity for all the health problems,” she noted.
LA PRENSA GRÁFICA sent emails this Wednesday to the Minister of Health, Dr. Francisco Alabi, and the director of the Social Security Institute, Dr. Mónica Ayala, to ask them about this issue, what the benefits of the merger would be if finalized, and how they are working to carry it out. At the time of publication, a response was pending.
STAFF REDUCTIONS
During the interview, Dr. Aguirre also referred to the lack of service assistants at the Social Security Institute, which overburdens other workers with their duties.
The reduction in assistants is 50%. In April, SIMETRISSS had already reported that the Social Security Institute would not renew the contract with the company that provides this service.
“It is not that we have a surplus of service assistants, nursing staff, or health workers; the problem is that we have a deficit, and on top of this deficit is the fact that our colleagues will now have to cover these areas, which are basically cleaning areas,” Aguirre explained.
“They are telling the nursing staff that while the service assistants are covering some areas, the nurses have to cover the ones the assistants cannot get to. They are degrading their work,” the union leader concluded.
Fusión ISSS y MINSAL “ya se está dando”, dice SIMETRISSS
El Sindicato de Médicos Trabajadores del Instituto Salvadoreño del Seguro Social (SIMETRISSS) dijo que la fusión entre la institución y el Ministerio de Salud (MINSAL) ya está ocurriendo, pues aseguraron que en el Hospital Amatepec, en Soyapango, se brindan citas y se realizan trámites a pacientes que llegan del sector público.
“Se puede decir que la fusión ya se está dando. Hace un año dijeron que no, que no había fusión, pero hoy vemos que hay presencia del ministerio de Salud en la emergencia del Hospital Amatepec, donde ya se está manejando la papelería del ministerio de Salud a nivel de usuarios”, dijo el doctor Rafael Aguirre, secretario general del SIMETRISSS, en la entrevista del martes 17 de junio en el espacio Informa TV.
El líder la gremial explicó que en el Hospital Amatepec el personal ya está utilizando el Sistema Integrado de Salud, un programa informático del MINSAL para atender a los pacientes. Agregó que la fusión no se ha completado en un 100 %, pues todavía se sigue solicitando el “derecho” a ser atendido en el Seguro Social.
“En este momento la emergencia del Hospital Amatepec ha echado andar el Sistema Integrado de Salud, que es un sistema informático que tiene el ministerio (de Salud) para poder atender a la población. Todavía se sigue solicitando el derecho, es decir, si la persona tiene o no tiene derecho al Seguro Social, pero una vez quitando eso podemos decir que la fusión se ha completado”, agregó el médico.
El representante de SIMETRISSS explicó que al hablar de derecho se refiere a que una persona es cotizante o beneficiario del Seguro Social, pues se les realiza un descuento de planilla (en el caso del cotizante), por lo tanto se le debe atender en cualquier clínica u hospital de la institución; sin embargo, en el Hospital Amatepec se están atendiendo a pacientes que no están inscritos en la institución.
“Las personas están llevando papelería (citas, órdenes de exámenes y otros documentos) al hospital (Amatepec) y lo primero que los compañeros dicen es ‘pero esa es orden del MINSAL’, entonces lo que hicieron es repartir un memorándum que dice que si las personas llegan con esa papelería que no les nieguen las citas, que las saquen”, subrayó el doctor Aguirre.
“Vemos nosotros que ya se está utilizando esa papelería (del MINSAL) y se ha solicitado que cuando lleguen (pacientes) a tramitar ultrasonografías, rayos X o lleven recetas que tengan el logo del ministerio de Salud se atiendan y se les dé cita a estas personas”, aseguró.
Eso, para el líder de SIMETRISSS, es ya una forma de fusión de ambas instituciones, aunque ninguna de ellas lo haya confirmado oficialmente.
“Esa, prácticamente, es una parte ya fusionada de ambas instituciones porque si la gente que lleva cuestiones del MINSAL puede ir a sacar cita a cualquier lado y el Seguro Social está llevándolo a cabo, eso ya está siendo fusionado. Lo único que falta para que digan que ya se hizo una fusión es que quiten la cuestión de solicitar el derecho o no de seguridad social que tiene el paciente”, añadió el especialista.
Aseguró que al momento solo en el Hospital Amatepec están recibiendo pacientes que no forman parte del Seguro Social para realizar citas u otros trámites, aclaró que en el lugar la atención médica se brinda, por ahora, solo a derechohabientes, es decir, a los cotizantes y sus beneficiarios.
Del mismo modo explicó que eventualmente este tipo de fusión se echará a andar en la Unidad Médica de Santa Tecla y en el Hospital Policlínico Zacamil.
El pasado 15 de abril, el Foro Nacional de Salud (FNS) se pronunció sobre la fusión del ISSS y el MINSAL y aseguraron que el objetivo de la misma es reducir los costos y que no garantiza que a la población se le dé un buen servicio.
“Si van a unir el sistema de salud, el MINSAL con el Seguro Social, yo lo estoy viendo más para ahorrarse personal y todo lo demás, no es para mejorar la atención, y si la van a mejorar, debería ser con estándares de calidad internacional, no solo por unir y reducir costos, sino por garantizar la atención de la población”, expresó Morena Murillo, del FNS.
En aquel momento, Murillo explicó que al hacer una fusión ISSS-MINSAL se debería definir qué institución hará promoción en salud, responsabilidad que recae, al momento, en el sector público.
“El Seguro Social no hace labores de promoción de la salud o prevención de la enfermedad. El 17 % de la población es atendida en el Seguro Social, entonces se le cargaría eso al MINSAL y no tendría capacidad resolutiva para todos los problemas de salud”, señaló.
LA PRENSA GRÁFICA envió este miércoles correos electrónicos al ministro de Salud, el doctor Francisco Alabi; y la directora del Seguro Social, la doctora Mónica Ayala, para consultarles sobre este tema, sobre cuáles serán los beneficios de concretarse la fusión, cómo están trabajando para llevarla a cabo. Al momento se está a la espera de una respuesta.
REDUCCIÓN DE PERSONAL
Durante la entrevista, el doctor Aguirre también se refirió a la falta de auxiliares de servicio en el Seguro Social, lo cual recarga en sus funciones a otros trabajadores.
La reducción de los auxiliares es de un 50 %. En abril, el SIMETRISSS ya había adelantado que el Seguro Social no renovaría el contrato con la empresa que provee este servicio.
“Nosotros no es que tengamos exceso de auxiliares de servicio, exceso de personal de enfermería, de trabajadores de la salud, el problema es que tenemos un déficit y sobre este déficit se agrega hoy el hecho de que los compañeros van a tener que cubrir estas áreas, que son prácticamente áreas de limpieza”, explicó Aguirre.
“Al personal de enfermería le están diciendo que mientras el personal auxiliar de servicio está cubriendo algunas áreas, ellas tienen que cubrir las que ellos no alcanza a hacerlo. Están degradando su trabajo”, concluyó el líder de la gremial.
